Welcome to COCA
Everyone entering the health care system is entitled to receive qualified professional, coordinated care throughout the healthcare system regardless of the facility that they are entering. The Northwest Indiana Continuity of Care Association was formed to enhance the concept of continuity of care as (a) an important part of the health care continuum and (b) as an integral part of total patient care experience.
The goal of the Northwest Indiana Continuity of Care Association is to serve as advocates for legislation and a voice for the community relating health concerns and issues by identifying and communicating them to members. Working in tandem with other allied organizations, we support the standards of professional competence which identify, structure, and promote a multi-disciplinary approach to continuity of care.
