We are a concerned group of professionals that have joined together to reduce care transition issues and promote improved outcomes. By working together we can effectively provide patients the most effective, safe and beneficial care possible.


We offer a hub of organization links, informational documents, government resources, regulatory information, and many other important member resources. We are here for our members to ensure they are armed with the information they need.

Welcome to COCA

Everyone entering the health care system is entitled to receive qualified professional, coordinated care throughout the healthcare system regardless of the facility that they are entering. The Northwest Indiana Continuity of Care Association was formed to enhance the concept of continuity of care as (a) an important part of the health care continuum and (b) as an integral part of total patient care experience.

The goal of the Northwest Indiana Continuity of Care Association is to serve as advocates for legislation and a voice for the community relating health concerns and issues by identifying and communicating them to members. Working in tandem with other allied organizations, we support the standards of professional competence which identify, structure, and promote a multi-disciplinary approach to continuity of care.
