Indiana Continuity of Care offers monthly meetings, including area chapters throughout the state as well as an annual professional conference for case managers, social workers, administrators and care coordinators. All meetings offer continuing education credits and great networking opportunities with new resources. Membership also includes a monthly update on healthcare happenings.
